Screening Process
Volunteer
mentors are required to undergo the same criminal background and health checks
that school employees are subject to prior to employment. The following screening
process must be undertaken before being interviewed for matching with a student:
- Submit mentor
application*.
- Provide copy of
drivers license, proof of auto insurance, 3 personal references*.
- Get fingerprinted
at a livescan site.
- Attend mentor
training (2.5 hours).
- Undergo TB test/reading
or chest x-ray*.
Chest
X-ray for TB:
Mail
physician’s letter* verifying that x-ray is negative for TB.
TB
Testing:
- If you have been
previously tested - If you have had a TB test within the last 4 years, you
need only provide a copy* of the results from your health practitioner.
- If you need a
new TB test/reading - remember, the reading must be done within 72 hours of
the test or you will need to be re-tested:
- Your own personal
physician or HMO can be utilized.
- Call your County

schedules for free testing/reading are available.
- Call Students

about specific TB test programs that exist in select communities.
